Welcome to the admissions area for the Doctor of Pharmacy (Pharm.D.) program! We are pleased that you are interested in admission to our program.  To learn more about the UW-Madison School of Pharmacy, be sure to view our video about the history of the School of Pharmacy.

Note that Fall 2012 Pharm.D. applicants must submit a PharmCAS application and a UW-Madison Supplemental application - both due on Nov 1, 2011 (Priority Deadline is October 1, 2011).

NOTE: Currently, UW-Madison has a standard application process (no rolling admissions).  We are piloting a process, beginning with Fall 2012 admission, which will allow for small group of applicants to be accepted in January. To be eligible for early consideration, all application materials must be submitted by the priority application deadline of October 1, 2011. Students not accepted in January will be notified of the admission decision in March.
