• Applicants may apply to multiple sites in our residency program for one application fee.
• If the system doesn’t allow you to submit for multiple sites within our program, just submit for one site, but indicate all of the sites within our program that you are interested in on your application. We will then share documents to all the sites you requested.

Applicants MUST meet the following requirements:

1. Hold a Doctor of Pharmacy degree from an ACPE accredited school
2. Have prior clerkship experience in the provision of advanced patient care services in a community pharmacy setting
3. Be eligible for licensure in Wisconsin
4. Be a U.S. Citizen or a permanent resident of the U.S.
