The Community-Based Pharmacy Residency program features a variety of innovative community pharmacy practice sites in the cities shown below. Detailed descriptions of these sites can be found further down on this page. The University of Wisconsin–Madison School of Pharmacy is the administrative hub of the Program.

Our program is accredited by ASHP and each site has its own unique National Matching Program Code Number.

Boscobel Pharmacy, Boscobel

Boscobel Pharmacy is an independently owned pharmacy offering advanced patient care services. We employ an experienced staff of pharmacists, technicians, bookkeepers, and pharmacy assistants to assure a high level of patient care.

Our patient care services include synchronized pick-up, medication management, new to therapy follow-up, immunizations, sterile and non-sterile compounding, and full line DME including diabetic shoes and compression, CPAP, oxygen, enteral care provision.

We maintain several collaborative practice agreements and work closely with our local clinic and hospital providing various forms of consultation. We look forward to training the next generation of community pharmacists and accept students from the University of Wisconsin-Madison, Concordia University, and University of Iowa.

Forward Pharmacy, Cottage Grove

Forward Pharmacy is an independent pharmacy in Wisconsin founded in 2017 with six locations in the Madison area: Cambridge, Columbus, Cottage Grove, Deerfield, McFarland, and Monona. The pharmacists, technicians, and associates at each of our stores are part of the community. We offer the kind of service your grandparents received at the local drugstore combined with the cutting-edge practices of modern pharmacy.

We pride ourselves on being at the forefront of the field of pharmacy, while also offering personalized care to each of our patients. Mercy Walworth Pharmacy (Lake Geneva, WI)Mercyhealth East Pharmacy is a top destination for advanced community-based pharmacy training and exceptional patient care. Located in Janesville, Wisconsin, our outpatient pharmacy is embedded within a multi-specialty clinic, offering a dynamic environment where clinical and community pharmacy teams collaborate to improve lives.

Our program emphasizes hands-on learning with pharmacy leadership and specialists in endocrinology, ambulatory care, and specialty pharmacy. Residents engage in quarterly supervisor meetings, educational sessions with medical residents, and system-wide implementation initiatives.

Streu’s Pharmacy Bay Natural

Streu’s Pharmacy is an independent community pharmacy and long-term care pharmacy founded in 1954. Our mission is to deliver the highest quality pharmacy services above and beyond dispensing medications, with an emphasis on compassion and personal connection; a commitment to innovation, training and expertise; for every individual need, no matter how complex; and in a way that reinforces our reputation for excellence with customers, prescribers and the community. We service a majority of the area’s assisted living facilities and a wide range of ages and socioeconomic statuses and employ almost 100 employees including pharmacy technicians, nurses, and 11 pharmacists.

The pharmacy resident will gain experience and expertise in chronic disease state management, psychiatry & long-acting injections, vaccines, geriatrics, pharmacogenomics, point of care testing, health and wellness services, leadership/administration, precepting for three area schools, medical billing, non-sterile compounding, DME, and so much more! We maintain over 10 collaborative practice agreements with our medical director and are a top performing pharmacy with the Pharmacy Society of Wisconsin and the Community Pharmacy Enhanced Services Network
