Flu Vaccine Clinic for SOP students and staff!

• Operation Immunization will be partnering with VaxPro to provide flu vaccinations to ALL pharmacy students, faculty, and staff on September 25th from 10:30 AM – 1:30 pm in Rennebohm Hall Room 2121.
• Please fill out the form using the link below to book an appointment. 
• Please have your insurance card handy when completing the form. 
• Flu shots are available at no cost to students.
• Get vaccinated to protect yourself and our community against the flu! 
• PharmD Students:  After your vaccination, you will be given a receipt to upload into the health portal.  Ensure your name is on the documentation.
