Title: Advancing Models of Care Through Pharmacist–Clinician Partnerships
This presentation will explore how pharmacist–clinician partnerships can expand access to care and support new models of team-based care, particularly for chronic disease management. Drawing on examples from primary care, the presentation will highlight practical strategies, lessons learned, and opportunities to integrate pharmacists more fully into patient care.
About the Speaker:
Dr. Korey A. Kennelty, PharmD, MS, PhD, BCGP is an Associate Professor in the Department of Pharmacy Practice and Science in the College of Pharmacy and the Department of Family Medicine in the Carver College of Medicine at the University of Iowa. Her clinical practice is through the University of Iowa Hospital and Clinics as a geriatrics pharmacist. Dr. Kennelty received her B.B.A. (2001) from the University of Toledo, Pharm.D. (2008) from Midwestern University- Glendale, and M.S. (2010) and Ph.D. (2013) from the University of Wisconsin-Madison. She then completed a 3-year Advanced Fellowship (2016) at the William S. Middleton Memorial Veterans Hospital specializing in geriatrics and implementation science.
