School of Pharmacy Citation Award Nomination

Criteria for the School of Pharmacy Citation Award

The School of Pharmacy Awards and Citation Committee invites you to nominate distinguished alumni or friends of the school for the UW School of Pharmacy Citation Award. Citations are awarded in the categories of education, practice and industry. Only an honorary degree constitutes a higher honor than a citation.

The award honors distinguished alumni or friends of the school who have established a record of public service well beyond outstanding professional practice alone, and to persons who hold a degree from the school or the university, or who otherwise have been closely identified with Wisconsin at some stage of their career, and who have an outstanding record of contribution to pharmacy and society through research, teaching or involvement in professional and community organizations. School of Pharmacy alumni, faculty, staff and professional colleagues may submit nominations.

Submit a Nomination

Complete nominations include:

  • A letter of nomination and a detailed outline of the nominee’s qualifications.
  • Nominee’s curriculum vitae and contact information (current address and phone number)

Deadline: Nominations for the citations should be received no later than March 1 of each year.

Alumnus and Young Alumnus of the Year Award Nominations

The Alumnus of the Year award is given to a member who has gone above and beyond for the field of pharmacy, their community and/or the UW–Madison School of Pharmacy. Nominees may have achieved a high honor or distinction as a pharmacist, made significant contributions to community efforts or organizations and/or assisted in the promotion or advancement of the Pharmacy Alumni Association over a sustained period of time. The candidate should have demonstrated this dedication over the duration of their career, leaving a lasting effect on the profession, community, and/or School of Pharmacy.

The Young Alumnus of the Year award is given to a member who has made great strides early in their career towards making an impact on the profession and/or school. They exemplify the standards of the UW–Madison School of Pharmacy and uphold its reputation in all areas. Candidates must have graduated within the last 15 years to be considered for the award. Nominees are well on their way towards becoming a candidate for Alumnus of the Year.

PharmTox Alumnus of the Year Award Nomination

PharmTox alumni are leaders in healthcare, academia, industry, entrepreneurship, government, and law. The Pharmacology and Toxicology Alumnus of the Year award, sponsored by the UW–Madison Pharmacy Alumni Association (PAA), honors an alum whose exceptional professional achievements have brought distinction to themselves, credit to UW–Madison, and benefit to their vocational community and/or residential community (e.g., town, city, state), in any career field.

The recipient will be recognized at the PharmTox graduation celebration during Commencement and will have the opportunity to speak to the PharmTox graduating class and their honored guests.

Nomination Eligibility and Criteria

To be eligible for nomination, the person must be a graduate of the Pharmacology and Toxicology undergraduate program at the UW–Madison School of Pharmacy.

To nominate a member of the alumni for the award, we ask that you submit a one-page nomination letter that describes the following:

  • What are the unique achievements that have brought distinction to the nominee and impacted their profession? In what ways have the nominee’s achievements truly been extraordinary? Please describe and interpret the nominee’s accomplishments in terms that a lay reader can understand.
  • What are the recognized contributions made by the nominee that demonstrate a benefit to the larger community and/or service to society? How has the nominee exemplified the Wisconsin Idea?

You may nominate multiple individuals, including yourself. Nominations submitted in prior years will automatically be reconsidered for this year.

The annual deadline for nominations is October 15 of each year.
