School of Pharmacy UNIVERSITY OF WISCONSIN - MADISON Information for Admitted Students Applicants admitted to the PharmD program must be prepared to meet all requirements listed below before matriculation. Please contact the Admissions Office with any questions at email@example.com or 608-262-6234. • Provisional Admission All Offers of Admission are provisional in that failure to complete all necessary post-admission requirements can result in admission revocation. All conditions are outlined in the admission offer documents and may vary depending on the student. For example, failure to complete the required Certiphi background check, failure to submit required immunization records, failure to complete remaining pre-pharmacy coursework are all examples of post admission requirements that may result in admission revocation. • Admission Deposits (Two deposit payments totaling $500) All admitted candidates to the School of Pharmacy will be required to submit a $200 admission deposit at the time of acceptance. An additional deposit of $300 will be required after March 1. Students admitted after March 1 will be required to submit the full $500 deposit. All deposits will be credited to the first semester tuition upon enrollment. The deposit will be forfeited for students not enrolling in the School of Pharmacy for the term in which they were admitted. Learn about future tuition updates. • In-Progress Course Requirements All in-progress and future pre-PharmD courses must be completed with a grade of B or better. • Multiple Offers Accepted PharmCAS Schools receive reports from PharmCAS (PharmCAS Shared Offer Reports) indicating those accepted students with multiple acceptances at other institutions. The UW-Madison School of Pharmacy reserves the right to request that accepted applicants commit to UW-Madison by March 1. If a student refuses to decline offers from other programs by March 1, the UW-Madison School of Pharmacy has the right to revoke the offer of admission. • Criminal Background Checks The University of Wisconsin School of Pharmacy requires that admitted students undergo a criminal background check prior to contact with patients. Students are required to submit background check results from our company of choice at the time of admission and during the DPH-3 year. The student is responsible for all background check fees. Problematic background check results may result in significant limitations in practice experience options. Because practice experiences are part of the program graduation requirements, the ability to graduate might be compromised. Students have the ability to react to, and explain background check results. If admitted, you will receive additional information about the background check process. • Health Documentation and Immunizations Students are required to have current immunizations and/or tests as a condition of enrollment. Please note that some tests such as the two-step TB test, can take several weeks to complete. Please see the School of Pharmacy’s Health policy for specific details. • American Heart Association’s Healthcare Provider CPR course The University of Wisconsin School of Pharmacy requires all PharmD students to complete the American Heart Association’s Healthcare Provider CPR course and to maintain this certification throughout the program. Incoming PharmD students are encouraged to take a CPR class here at the School of Pharmacy during orientation. Students are responsible for the cost of the CPR course. • HIPAA & Safety Infection and Control Training All PharmD students must complete the HIPAA (Health Insurance Portability and Accountability Act) training as well as the Safety Infection and Control Training prior to beginning the PharmD program. Additional information in regards to these trainings will be available on the Admissions Checklist in the spring. • Health Insurance All UW-Madison students are eligible for SHIP (Student Health Insurance Plan) through University Health Services for an additional fee. Health insurance is not required for domestic students. All international students and visa dependents are required to have UW–Madison approved health insurance coverage. International students must enroll in SHIP or file a qualifying waiver by the compliance deadline unless you qualify for an automatic waiver. • Housing The majority of PharmD students live in off-campus housing near the School of Pharmacy with other pharmacy colleagues. • Physiology Exemption Exam Admitted students who have completed and earned a B or better in a stand-alone Human Physiology course (1 semester) or a combined average of a B or better in an integrated Anatomy and Physiology course (2 semesters with lab) from colleges/universities other than UW-Madison will have the opportunity to complete a standardized Physiology Exemption Exam during orientation. The exemption exam is rigorous, but if students pass the exam, they will be exempt from taking Physiology 335 during the first semester of the PharmD program. Admitted students will receive an email with additional details in regards to the exemption exam in May. • Technical Standards All applicants accepted into the University of Wisconsin PharmD program need to meet the Technical Standards with or without reasonable accommodations. • Student Promotion Policies and Procedures & Student Alcohol and Other Drug-Related Policies All admitted students must read and sign the Student Promotion Policies and Procedures & Student Alcohol and Other Drug-Related Policies. Students must adhere to these Policies while enrolled in the School of Pharmacy. • Computer Policy All Doctor of Pharmacy students are required to have access to a Windows or Macintosh laptop during class with the required hardware and software specifications. • New Student Orientation All newly admitted PharmD students will be required to attend a mandatory New Student Orientation. The New Student Orientation for students accepted for the Fall term will be held a week prior to the start of the semester. More details will follow. • DPH-4 Year – Cost and Travel Considerations Fourth year PharmD students must also pay tuition and mandatory fees for the summer semester, as the fourth year consists of three semesters of clinical rotations taken for academic credit. During your fourth year at the UW-Madison School of Pharmacy we strive to provide our students with a thorough education that includes a broad variety of experiential learning opportunities. Many of these can be guided by your preferences and selections; others will be the result of the match process or faculty assignment. Regardless, it is a good idea to prepare for expenses in addition to traditional tuition, room and board during this year. APPE sites are located throughout the state of WI. The rotations will challenge you and provide you with a full range of patient populations and pharmacy practices to hone your skills and explore your interests. It will be your responsibility to arrange transportation to and from your IPPE/APPE sites. It is highly recommended for convenience purposes that you have a driver’s license and a car by the 4th year of your program. If you don’t have a car, you will need to find alternative transportation such as borrowing a car from a family member or friend, getting a ride from a colleague, utilizing a zip car, renting a car, using the city bus, and/or checking out ride boards. Rotations are offered in different regions throughout Wisconsin. The regions are as follows: Eau Claire, Green Bay, La Crosse, Madison, Milwaukee, Wausau. Students choose which region to practice, but please note that not all students are able to practice in the Madison region. Twelve months of practical pharmacy experience will strengthen your skills, increase your knowledge, and build your confidence. You will have the opportunity to actively provide care for patients under the supervision of the pharmacist preceptor in the hospital and community pharmacy settings. Given the nature of this experiential year, there are additional costs that occur. Added expenses may arise when arranging transportation, room and board at various locations; acquiring the necessary health records to work in a pharmacy setting; obtaining essential badges or the identification needed at some sites; and costs related to generating and copying required documents. Should you choose to participate in an APPE Resident Track, you will have the opportunity to refine your skills in a specific field. You will realize tremendous professional growth and will graduate with greater competency to begin your pharmacy career. APPE RT’s do have financial considerations, however. You may incur costs related to your research project as well as those for making the poster. You will also be encouraged to travel and attend the ASHP Midyear Clinical Meeting or other national conferences to present your work. Your fourth year also provides the discretionary opportunity for international travel to gain global pharmaceutical experience. The costs for these programs vary depending on the country and plan selected but will include expenses due to the travel, housing, food, passport/visa, insurance, immunizations, and medication that may be required for the trip. You will want to carefully consider these variables when planning your DPh-4 options. Your experiential education at the UW-Madison School of Pharmacy will be both rigorous and rewarding, providing the base of your future career. Make it as powerful as possible by planning ahead and giving careful thought to the expenses that could be involved. We are looking forward to providing a comprehensive array of options and tools so that you can attain your professional goals. • International Student Employment Considerations If you are an international student, it is important to know that the University of Wisconsin-Madison School of Pharmacy does not guarantee residency or employment after graduation. In recent years, it has become increasingly challenging for international students to obtain employment or a residency within the United States. In order for international students to obtain employment within the U.S. after graduating from the Doctor of Pharmacy program, they MUST find an employer who is willing to sponsor them through a work visa. This issue is not unique to the University of Wisconsin-Madison or the State of Wisconsin. • AACP Cooperative Admission Guidelines In an effort to allow applicants the time and ability to explore multiple pharmacy programs, the following Big Ten programs are participating in the AACP Cooperative Admissions Guidelines (CAG’s) Ohio State University University of Illinois at Chicago (UIC) College of Pharmacy University of Iowa University of Michigan University of Minnesota University of Nebraska University of Wisconsin There may be other programs participating in the CAGs; the PharmCAS School Pages provide a directory of programs. Prior to March 1, these programs will not require a deposit of more than $200. Applicants to these programs may accept offers from multiple PharmD programs until March 1, at which point they may hold only one acceptance. If applicant hold more than one acceptance, programs may choose to rescind admission offers. After March 1, applicants may continue to interview. If offered admission to a program after March 1, an applicant who had previously accepted another program’s offer must withdraw from that program in order to accept the new offer. Be sure to check with each program on their specific deposit amounts and timelines. • Campus Safety Report The 2017 Annual Security Report and Annual Fire Safety Report contains current campus safety and disciplinary policies, crime statistics for the previous three calendar years, and on-campus student housing fire safety policies and fire statistics for the previous three calendar years. UW-Madison will provide a paper copy upon request; please contact the University of Wisconsin Police Department at firstname.lastname@example.org or 1429 Monroe St., Madison, WI. • Wisconsin Caregiver Background Check The State of Wisconsin “Caregiver Background Check Law” (“Caregiver Law”) went into effect on October 1, 1998. The law requires that certain individuals who provide care for others or have access to clients of a covered entity* must undergo a criminal background check. The Wisconsin Department of Health Services has ruled that professional health care (e.g., pharmacy, medicine, nursing, and social work) students are subject to the law, because of the experiential course work (clerkships) in their academic programs. Accordingly, it is necessary that a condition of PharmD admission be completion of the Wisconsin Department of Justice “Background Information Disclosure” form. If admission is approved, a “Background Information Disclosure” form will be available on the PharmD Admissions Checklist. It will be imperative that an admitted student fill out the “Background Information Disclosure” form completely. If an admitted student has lived in one or more states other than Wisconsin during the past three years, a criminal background check will need to be conducted in each such state. If an admitted student has been discharged from any branch of the military during the past three years, the student will need to provide his/her military discharge papers. Background checks are conducted by a national background screening company on behalf of the School of Pharmacy via the background company’s on-line order system. The results of the background check are returned to the applicant, and a copy is sent to the School of Pharmacy for review. The information obtained by this check will be used by the program’s experiential/clinical training facilities to determine the individual’s eligibility to train within the facility. The “Caregiver Law” identifies numerous “serious crimes” that automatically bar a student from participation in clerkship experiences, unless the student successfully completes the Wisconsin Department of Health and Family Services rehabilitation review process. For all other offenses, it will be the prerogative of a prospective clerkship site supervisor to accept or not accept a student as a clerkship participant, based upon whether the offense in question substantially relates to the care of the site’s clients. If a student is not eligible to participate in the required clerkship experiences, the student will not be able to earn the PharmD degree.
