The COPD CARE service is a comprehensive, interprofessional transitions-of-care service that
incorporates best practices outlined in the 2022 GOLD Report (GOLD Guidelines for COPD management) into COPD management during transitions from the hospital or emergency department to their homes following a COPD exacerbation. The COPD CARE service utilizes ambulatory care clinical pharmacist practitioners to make these best practices the standard of care for patients with COPD!

- Further enhance entrepreneurship and project management skillsets
- Create quantitative and qualitative data collection tools
- Questionnaires
- Interview guides
- Dashboard development
- Write and publish manuscripts to share COPD CARE assessment findings
- Develop poster and podium presentations at local, national, and international conferences
