The Awards and Citations Committee is seeking nominations for candidates for 2017 UW Citations. Each fall as part of the alumni weekend celebration, the School of Pharmacy presents University Citations of Merit to four distinguished alumni or friends of the school who have:
made significant contributions to pharmacy/the pharmaceutical sciences and society through research, teaching, or involvement in professional organizations, and/or
provided substantial financial or other support to the school

Citations may be awarded to:
– A pharmacist who has an established record of public service well beyond outstanding professional practice alone; or
– A person outside the state who holds a degree from the School or University, or who otherwise has been closely identified with Wisconsin at some stage of his or her career, and who has a record of outstanding achievement.

To nominate an individual, please submit:
– a letter or email of nomination for each candidate, including the individual’s name and address
– information identifying his/her achievements and current professional status
– why you believe he/she should receive a Citation.

Please include the nominee’s contact information, including email address. If available, a current electronic curriculum vitae or resume of the nominated individual is also appreciated.
