Preclinical
  1. Study director seeks feedback about intervention from a Pharmacy Management Advisory Group.
  2. Pharmacy managers identify intervention teams (1 pharmacist, 1–2 pharmacy technicians per site)
  3. Investigators provide 8 h of TEAM training (including self-study):
    • Module 1: TEAM tools and guidelines for measuring and managing BP in Black patients
    • Module 2: TEAM tools and guidelines for improving patient adherence and involvement
    • Module 3: TEAM tools and guidelines for patient follow-up and physician collaboration
  4. Investigators provide portable furniture, supplies, and guidelines, including folding table, chairs, privacy screen, BP monitor and cuffs, JNC card, patient charts with tools, guidelines, and algorithms.
  5. Screeners recruit, screen, and enroll eligible patients. Researchers provide list after goals are met.
Patient visit 1
  1. Pharmacy technician makes, confirms, and reschedules all appointments as needed, sets up furniture, prints profiles, welcomes patient, measures BP, and asks patient to fill out previsit tools:
    • Brief Medication Questionnaire (BMQ-H9): 9-item checklist for eliciting patient feedback regarding adherence and barriers to adherence
    • Health Checklist (HC): 3-page checklist for assessing patient history, lifestyle, and BP goals
  2. Pharmacist reviews previsit tools, collaborates with patient in barrier reduction, distributes and uses take-home kit to enhance patient understanding and involvement, and completes TAP.
    • Patient take-home kit: 7-day pillbox, My BP Tracker, and easy-to-read leaflets “Know Your Numbers” and “Manage Your Medications”
    • TAP: 1-page tool for noting BP, barriers, interventions, plans, and time
Patient visits 2–6
  1. Pharmacy technician confirms appointment, sets up furniture, prints profile, welcomes patient, measures BP, and asks patient to fill out the top part of BGC.
    • Brief Goal Check (BGC): Patient completes patient section of checklist for assessing adherence, barriers to adherence, and progress since the last visit.
  2. Pharmacist reviews BP and patient feedback, collaborates with patient in barrier reduction, provides reinforcement and support, documents interventions/progress, and contacts MD if necessary.
    • Brief Goal Check (BGC): RPh completes provider section of checklist for assessing adherence, barriers to adherence, progress, and plans.
    • Request for Medication Review (RMR): 1-page tool for faxing MD, if necessary
Payment
  1. Employer receives compensation for up to 6 documented TEAM visits per eligible patient.
