Welcome to the admissions area for the Doctor of Pharmacy (Pharm.D.) program! We are pleased that you are interested in admission to our program.  To learn more about the UW-Madison School of Pharmacy, be sure to view our video about the history of the School of Pharmacy.

The application for Fall 2013 is closed.  The Application for Fall 2014 will be open in the Summer of 2013.

To ensure that your application process goes smoothly, please carefully review all of the information in the admissions area prior to applying.  Prospective students are also able to contact our Pre-School of Pharmacy advisor to learn more about preparing for admission to the program.  Call (608) 262-6234 or .

Note that Fall 2014 Pharm.D. applicants must submit a PharmCAS application and a UW-Madison Supplemental application - both due on Nov 1, 2013  (Even though we will accepted all applications submitted by the November 1 deadline, we encourage you to submit your PharmCAS application at least three weeks prior to the deadline.  This will allow us to begin reviewing your application close to the deadline).
