Resident Jeff Freund works with a patientThe University of Wisconsin-Madison Ambulatory Care Pharmacy Residency Program (UWACPRP) presents a unique opportunity for the PGY-2 candidate to provide medication therapy management for patients receiving care in a community health center medical home environment, to engage in formal teaching of  pharmacy students at the School of Pharmacy and in the practice setting, to participate in the education of family practice residents, and to conduct practice-based research in collaboration with pharmacists and other health care providers in the community setting.

Purpose Statement

The purpose of the PGY2 Ambulatory Care Residency Program is to train pharmacists for clinician/teacher positions in academic and/or ambulatory care environments.  Successful candidates will have the knowledge, skills and attitudes to assume leadership roles in ensuring high quality medication therapy management as an integral member of an ambulatory healthcare team, and the ability to teach/mentor colleagues, students, and patients about evidence-based, safe, and effective use of medications.
